Heparin is a glycosoaminoglycan that is a naturally
produced substance. It has an anticoagulation effect by inhibiting the
formation of fibrin clots. Heparin acts primarily at the following sites
in the normal coagulation system

Combines with antithrombin III (heparin
cofactor) to accerate the neurtralization of thrombin which prevents
conversion of fibrinogen to fibrin

Inactivates factor X which prevent conversion
of prothrombin to thrombin

Prevents the formation of a stable fibrin
clot by inhibiting the activation of fibrin-stabilizing factor.
Heparin does not have fibrinolytic activity,
therefore, it will not lyse existing clots.
